Activities Release & Waiver of Liability Forms

Oceanside Community Association (OCA) offers the residents various activities in the common area clubhouse facilities. To enable OCA to continue to offer these activities and encourage the volunteers’ participation, a certain level of protection is needed.

The OCA-sponsored activities listed below require that a release and waiver of liability be signed by all volunteers and participants.

Please print out the form, fill in your name, unit and house number, and phone.  Then submit it to the appropriate activity leader prior to your participation.

Activities Reimbursement Requests

All requests for reimbursement for activities-related expenses must be pre-approved by the Activities Committee (AC) at a regularly scheduled meeting.  You must submit your request to the Social Director one week prior for presentation at the AC monthly meeting.
